Community Habilitation Services are designed to support individuals of all ages with disabilities to learn or improve skills and functioning for daily living to achieve independence in their lives, home, and community. STIC provides person centered services in a 1 staff to 1 individual ratio, in the community or consumer’s home, at the times that best meet the consumer’s needs. Community Habilitation services are goal oriented and offer skill training and supports as follows: daily living skills, travel, health, adult educational supports, communication, social skills, leisure skills, money management, positive behavioral supports, and safety skills.
Eligibility
A person must meet eligibility criteria as defined by the New York State Office for People with Developmental Disabilities (OPWDD). A referral should be made by your Care Manager.

The Front Door staff will guide you through the steps involved in finding out if you are eligible for services with OPWDD, identify your needs, goals and preferences, and help you work on a plan for getting those services, including Community Habilitation.
