Open Doors / MFP
STIC’s Open Doors Program empowers individuals in nursing facilities, OPWDD Intermediate Care Facilities, or hospitals to transition to independent community living or smaller OPWDD group homes through personalized transition planning and advocacy. Transition Specialists provide ongoing support and skill-building for up to 12 months post-transition, ensuring sustained independence.
Traumatic Brain Injury (TBI) Waiver Regional Resource Development Center (RRDC)
STIC is the Regional Resource Development Center for the Department of Health Traumatic Brain Injury (TBI) Medicaid Waiver Program for the Southern Tier area of NYS. Our staff facilitate access to services and enrollment for eligible individuals.
Nursing Home Transition and Diversion Waiver (NHTD) Regional Resource Development Center (RRDC)
