OPEN DOORS
The Open Doors Program at STIC is dedicated to empowering individuals residing in nursing facilities, OPWDD Intermediate Care Facilities (ICF), or hospitals to transition back to independent community living or to smaller OPWDD Individualized Residential Alternatives (IRAs) with four beds or fewer. Through a person-centered approach, our Open Doors Transition Specialists work closely with participants to create safe, tailored discharge plans, coordinating with families, friends, Medicaid, social workers, residence managers, and community service agencies to ensure a smooth and successful transition.
Our Transition Specialists provide comprehensive support by conducting intakes, advocating on behalf of participants, and offering guidance to address individual needs throughout the transition process. For the first 12 months following a transition, Open Doors maintains regular contact with eligible participants, delivering ongoing support and skill-building assistance to promote sustained independence and community integration.
