SOCIAL CARE NETWORK
STIC is part of the Social Care Network (SCN) that connects New York residents receiving Medicaid to vital resources through a dedicated team of Social Care Navigators. Our Navigators conduct comprehensive screenings to identify unmet Health-Related Social Needs (HRSN), such as challenges with nutrition, housing, transportation, or social and behavioral health support. By fostering a person-centered approach, the program empowers eligible individuals to access tailored services that address their unique social, behavioral, and physical health needs, promoting overall well-being and greater independence in their preferred community settings.
STIC’s Social Care Navigators work closely with participants to link them to a wide range of community-based services, including meal programs, housing assistance, accessible transportation, and social care management, support that goes beyond traditional healthcare. Our team provides personalized guidance, and follow-up to ensure that services are effectively meeting participants’ needs, helping them overcome barriers to health and stability. Rooted in STIC’s commitment to independent living, the Social Care Network Program is designed to enhance quality of life by addressing the social determinants of health.
Eligibility -
For Enhanced Services:
1) Demonstrate 1 or more unmet social need.
2) Be enrolled in a Medicaid Managed Care Plan.
3) Meet the criteria of 1 or more enhanced service population.
For Level 1 Services (Existing Federal, State, and Local Services):
1) Must be authorized to receive New York State Medicaid.
2) Reside in NY State.
3) Do not meet criteria for Enhanced Services.
STIC’s Social Care Navigators provide screenings either in person or over the phone.
